 Disk questions

Hey all i'm getting some lines of errors dealing with my IDE hard disk. Here's what it looks like.
"ad0:UDMA ICRC WRITE ERROR"
"blk# 393295 retrying"
"blk# 524431 retrying"
"blk# 524431 retrying"
"blk# 524431 retrying"
"blk# 524431ata0-master: Warning: WAIT READY active=ATA_ACTIVE_ATA"
"falling back to PIO mode"

Does this mean my disk is bad? Also i get some fragmentation errors during the fsck check at bootup

"/dev/ad0s1a 0.6% fragmentation"
"/dev/ad0s1h 0.5% fragmentation"

should i worry about this? and how should i correct these problems?
